Feedback Form Script Demo
Introduction
This is a demo of the feedback form script generated by thesitewizard.com's Free Feedback Form Script Wizard (PHP and Perl Form to Mail Script). Or more accurately, this is a testing site, that I created to make sure that my feedback form script works on various free web hosts, including this one.

Some Items to Note
The form will send your email message to an unmonitored email account so don't use the form below to send me any real feedback. No one will read it. The reason for this is obvious - I doubt many of you using this form will want to spend time figuring out something polite to say to me when your real reason for using this form is merely to test the script. Since there's no point for me to read hundreds of "asdf asdf asdf asdf" messages, the script simply sends it to a black hole account.
The advertising you see on this page is not generated by the script. Both the PHP and Perl versions of the feedback form script created by thesitewizard.com are ad-free - they do not have any advertisements on it. The advertisements on this page are placed by the free web host on which this test script is hosted. If you put your form and script on a commercial web host, you will not get advertisements (unless of course you place them yourself).
Also remember free web hosting accounts like this have severe limits placed on script activity and bandwidth so if for some reason the script refuses to execute or you get some sort of error, it's possible that you are the unlucky person who happen to reach this site's limit this month.
The feedback form can look as pretty or as ugly as you want it to look. It's up to you to decorate your page. The Wizard does not force a certain appearance on your web page. The appearance of this page just happens to be the design I created for this demo site.
